Membership & Privileges

To apply for medical staff membership and privileges, please call the CSM Central Credentials office at 414.465.3737.  You will need to have your email address, date of birth, and social security number available when calling. 

The application process can take anywhere from 3-12 months before privileges are granted, so be sure to apply early!

Restraint and Seclusion

The Columbia St. Mary's Medical Staff Services Department provides meeting and CME program support for Columbia St. Mary's Medical Staff. The department also processes membership applications and privilege requests for all Columbia St. Mary's and affiliated hospitals, including CSM Milwaukee Hospital, CSM Ozaukee Hospital, Sacred Heart Rehabiliation Institute, Columbia Center and the Orthopaedic Hospital of Wisconsin.
